Our experienced Geriatric Care Managers are professionals who specialize in assisting mature adults and their families with long term care arrangements. Our Geriatric Care Managers have experience in Gerontology, Social Work, Nursing or Counseling.  Care Managers can 

• Conduct Care -planning assessments to identify problems, eligibility for assistance, and need for services;
   
• Screen, arrange and monitor in-home help or other services;
   
• Coordinate Medication on a weekly or monthly basis;
   
• Review financial, legal, or medical issues and offer referrals to geriatric specialist to avoid future problems and conserve assets;
   
• Provide Crisis intervention;
   
• Act as a liaison to families at a distance
   
• Assist with moving an older person to or from a retirement complex, care home or nursing home;
   
• Provide consumer education & advocacy; Offer counseling and support.
   

We provide cleaning, packing, organizing, minor home repair, yard services for those individuals who would like to spend their time and energy elsewhere. General cleaning tasks include vacuuming, dusting, mopping, laundry and linen change, bathroom and kitchen cleaning. Spring Cleaning includes heavy duty cleaning of all rooms. Our handy man can help with home maintenance, painting, and light carpentry.

Caring companionship at your home or during your personal outings or errands. Homemaking services for individuals who desire assistance with home management tasks; meal preparation, shopping and errands.

Bath visits and shifts up to 24 hours for clients who require personal care assistance; bathing, dressing, toileting and home exercise programs.  Short or long term assistance available. 

 Companions can accompany you to appointments, adult day programs, community centers, church services, shopping and social events.

To assist those who are busy with other activities. We help you remember Appointments, Special Family dates, Birthdays, Anniversaries, Medication and Treatment times. Daily visits and telephone reminders are available. We also provide a friendly phone companion to socialize, monitor safety and make sure other needs are being met.
